Common Risk Factors Prevention Model

Our mission is to improve the health of the Hispanic community. We believe that managing common risk factors is the best approach to decreasing the risk of developing chronic diseases. La Casa de la Salud (LCS) has adopted the Common Risk Factor Approach (CRFA), a framework used to guide cross-disciplinary health promotion programs for chronic diseases such as diabetes, heart diseases, stroke, hypertension, and cancer. Many of the common modifiable behavioral risk factors like lack of physical inactivity, poor diet, harmful use of alcohol consumption, tobacco use, overweight/obesity, stress, sleep deprivation, and social isolation have a detrimental effect on overall health.

This graph shows modifiable and non-modifiable risk factors for common chronic diseases. As we advocate for a healthy community, this model guides our efforts in educating the community on the management of common modifiable risk factors through our health promotion programs. Here at LCS, we empower our community to manage these modifiable risk factors for a healthier and better quality of life.
