Partnerships That Expand Access to Care

We collaborate with Bon Secours, St. Francis Family Medicine Clinic, and VCU Massey Cancer Center to strengthen referral partnerships and expand access to care for uninsured and Spanish-speaking patients. These efforts focus on:

Community Resources: Helping patients at Bon Secours Care-A-Van and St. Francis Clinic receive timely guidance and support.

Prenatal Care: Connecting Spanish-speaking patients to Medicaid, WIC, and transportation assistance to support healthy pregnancies and birth outcomes.

Cancer Care Navigation: Supporting VCU Massey Cancer Center patients in accessing resources and addressing practical care barriers.

Working Together to Connect Communities to Care

Through partnerships, La Casa de la Salud provides bilingual navigation, outreach, and follow-up to guide participants to the services they need. Using the Unite Us platform, we coordinate with partner organizations and deliver our own direct services, strengthening care coordination and helping community members successfully connect to care and resources.

Community Impact Stories

“AG was diagnosed with breast cancer and referred to LCS for support. Initial attempts to access low-cost clinics were unsuccessful due to eligibility requirements and missed appointments. With our help, she connected with VCU, began treatment (including chemotherapy and surgery), and received ongoing emotional and social support. Her navigator continues to provide guidance and connect her with Spanish-language support groups and other community resources.”
